Job Summary
We are a new private-duty home care agency seeking a compassionate and dedicated Registered Nurse (RN) to oversee client care and care professional operations. The RN will ensure high-quality care through client/home assessments, care plan development, and care professional (Certified Home Health Aides) supervision, in compliance with New Jersey regulations.

Responsibilities
- Collaborate with clients/designated family members, and colleagues to create and implement patient care plans
- Assist with hiring qualified and compassionate care professionals
- Train and oversee all care provided by Dharma Home Care care professionals
- Make site visits to meet current and prospective clients, conduct home assessments, and ensure care professional compliance with the care plan and best practices
- Document any changes in patient conditions and adjust care plan accordingly
- Serve as a liaison between clients, families, and the agency, addressing concerns and ensuring satisfaction
- Maintain accurate records of patient care and discuss observations with clients/family members
- Assist with gaining and maintaining Commission on Accreditation for Home Care accreditation
- Ensure compliance with all state regulations, agency policies, and accreditation standards
Qualifications
- Bachelor’s Degree in nursing, social work, gerontology, psychology, health and human services, or other related field of human services OR 2 years’ experience in geriatrics and/or care management required
- Active RN license in good standing with the New Jersey Board of Nursing.
- Previous experience as a Registered Nurse is preferred
- Excellent interpersonal and client care skills
- Valid driver’s license
- Ability to travel within Bergen County to conduct visits (reliable transportation required)
- First aid/CPR certification required

Compensation: $42.00 - $50.00 per hour
